Essential Documents to Bring for Your VA Medical Appointment

 

When preparing for your first VA medical appointment, it's crucial to bring the following essential documents to ensure a smooth and efficient process: If you do not have any of the documents, don't worry we can help you obtain your military records.

1. DD-214 or military discharge papers: This document confirms your service and is necessary for eligibility verification.

​

2. Government-issued photo ID: A driver's license, state ID, passport, or military ID is required for patient check-in and identity verification.

3. VA enrollment confirmation: Your enrollment letter from VA or your VA enrollment number if you have it. Benefits Authorization Letter confirming enrollment in the Foreign Medical Program (FMP).

4. Complete medication list: Include all prescription and over-the-counter medications with dosages and frequencies. If you do not have the prescription list or you do not know the name of the medication, please bring the empty medication bottle.

5. Current medical conditions: A written list of all diagnosed conditions, when they were diagnosed, and current treatment status.
